Personally-identifiable
information collected with your consent may include, for example, name, mailing
address, e-mail address, telephone number, facsimile number, user name,
password, social security number, tax ID number, user Id, any response you
provide to survey questions, information relating to any orders you place, or
any inquires you may make through our Web site.
This
site uses first-party cookies, however personally identifiable information is
not collected through the use of cookies, except as described in the
"Personally-identifiable Web Site Acitivity Monitoring" section below. Cookies
are used to determine whether a visitor is unique or whether the visitor has
viewed our site before. This site also uses cookies to keep track of user Ids
and passwords, to enhance navigation and for session management.
